Here, slope (m) = y2 - y1 / x2 - x1
m = 6-4 / 12-4
m = 2 / 8
m = 1/4

Equation, y - y1 = m(x - x1)
y - 4 = 1/4 (x - 4)
Also, y - 6 = 1/4 (x - 12)
